Qualifications

• Current nursing license in MA in good standing
• Graduated from an accredited and approved nursing program
• At least 1 year of hands-on medical experience with adult/geriatric population in various care settings

Responsibilities

• Use head-to-toe assessment to identify changes in patient condition
• Manage acute and post-acute care needs (CHF, COPD, wounds, IV therapy)
• Triage and manage urgent clinical situations in the home
• Perform medication reconciliation and safety monitoring
• Conduct comprehensive wound assessment and documentation
