Qualifications
- Current, active nursing license in good standing.
- Graduate of an accredited nursing program with transcript or diploma.
- Minimum one year of hands-on medical experience with adult/geriatric population in healthcare settings.
Responsibilities
- Perform head-to-toe assessments and identify patient condition changes.
- Manage acute and post-acute home health care needs (CHF, COPD, wounds, IV therapy).
- Triage and independently manage urgent clinical situations at home.
- Conduct medication reconciliation, teaching and safety monitoring.
- Document wound care and patient progress thoroughly.
