Job summary

Mercy Home Health is seeking a compassionate RN to provide skilled nursing care in patients' homes across Missouri. You will perform comprehensive assessments, manage care plans, and collaborate with a multidisciplinary team to ensure high-quality, patient-centered outcomes.

Qualifications

• Experience: Prefer one year of experience in a related field (home health, acute care, etc.).
• Education: Graduation from an accredited nursing program required.
• Licensure: Current RN license in Missouri required. Must also have a valid driver’s license and proof of auto insurance.
• Certifications: BLS Certification required through the American Heart Association.

Responsibilities

• Provide day-to-day patient care in the home setting including case management and total care of multiple patients with varying acuity.
• Manage admissions to home health services and post-hospitalization assessments, recertifications, and discharges.
• Perform dressing changes, IV therapy administration, access site care, and general skilled nursing per policy and procedure.
• Document appropriately using the Epic computer system and coordinate care with the Mercy Home Health team.
