Job summary

BAYADA Home Health Care is seeking a Full Time Registered Nurse (RN) to join our home health visit team. You will be part of a multidisciplinary health care team delivering skilled nursing and rehabilitative care to clients so they can remain at home.

Responsibilities include head-to-toe assessments, triage of urgent cases, medication reconciliation, wound care, and comprehensive documentation. PA nursing license and 1+ year of relevant experience required.

Qualifications

• Active nursing license in PA
• Graduate of an accredited nursing program
• At least 1 year of hands-on medical experience with adult/geriatric population in acute, sub-acute, home health, hospice, ICU or med/surg settings.

Responsibilities

• Perform head-to-toe assessments and identify changes in patient condition.
• Provide skilled nursing and rehabilitative care in the home.
• Triage and manage urgent clinical situations in home settings.
• Conduct medication reconciliation, patient teaching and safety monitoring.
• Perform comprehensive wound assessment, care and documentation.
