Job summary

Nightingale Visiting Nurses is seeking Registered Nurses to provide home health care with direct patient care and case management. Positions include full-time, part-time, and per diem options, offering flexible schedules and compassionate, patient-centered care in patients’ homes.

Responsibilities include conducting admissions, creating care plans, coordinating with the care team, and ensuring HIPAA compliance. MA licensure, OASIS/WellSky experience, and CPR are preferred for this role.

Qualifications

• Active RN license in MA.
• Experience with OASIS and WellSky (Kinnser) strongly preferred.
• Med-surg and wound care experience a plus.
• Current physical exam within last 12 months.
• Current TB test within last 12 months and CPR certification.
• Valid driver’s license.

Responsibilities

• Provide direct patient care and case management per physician orders.
• Conduct patient admissions and develop care plans.
• Coordinate care with interdisciplinary team.
• Deliver skilled nursing care while maintaining HIPAA compliance.
• Document visit records promptly.
• Develop and update individualized care plans as needed.
• Coordinate referrals for therapy or medical social work.
• Evaluate nursing services and adjust as appropriate.
