Qualifications

• Current Registered Nurse license in Connecticut (CT) is required.
• OASIS and WellSky (Kinnser) experience preferred.
• TB test within 12 months and CPR certification required.

Responsibilities

• Conduct patient admissions and develop individualized plans of care.
• Obtain medical histories and perform physical assessments.
• Monitor patient status, vital signs and clinical progress in the home setting.
• Deliver skilled nursing care in compliance with HIPAA and agency standards.
• Collaborate with the interdisciplinary team to ensure comprehensive care and coordinate services.
