Job summary

Safe Life Home Health Care, LLC is seeking a skilled Home Health Registered Nurse to join our 4-star team in Chicago, IL. The RN will perform in-home assessments, develop care plans, administer medications, and monitor patient progress to prevent hospitalizations.

Ideal candidates have at least 1 year of home health experience, Illinois RN license, CPR certification, and proficiency with WellSky Kinnser EMR. We offer competitive compensation, benefits, and opportunities for ongoing education.

Valid Illinois RN license
CPR Certification mandatory
Valid Driver's License required
Auto Insurance required
Minimum 1 year home health experience
Proficiency in WellSky Kinnser EMR
Strong clinical assessment and communication skills

Responsibilities

Provide strong clinical skills, excellent communication abilities, and a passion for patient-centered care
Understands and adheres to established policies and procedures
Conduct comprehensive assessments of patients' health status and needs in their home environment
Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient’s physical or emotional condition
Develop and implement individualized care plans based on patient assessments and medical history
Administer medications and treatments as prescribed, ensuring adherence to safety protocols
Monitor patients’ progress, documenting changes in condition and response to treatment
Provide education to patients and families/significant others regarding disease process and management, medication regimen, pain management, fall prevention, infection control, proper diet / exercise, when to contact pharmacy / physician / SN and when to seek emergency care
Collaborate with interdisciplinary teams to coordinate care, including discharge planning and referrals to other services as needed
Maintain accurate medical records and documentation in compliance with legal and regulatory standards
Respond promptly to patient inquiries or concerns, providing support and guidance as necessary
Attends staff meetings, patient care conferences and in-services as scheduled
