Home Preferred Senior Care is seeking a Home Health Registered Nurse (RN) for a full-time role in Texas. The RN will perform initial evaluations, assess nursing needs, and coordinate plans of care for homebound patients.

Qualifications

• Current licensure in the state of Texas or a Compact license.
• Preferably 1 year of nursing experience in a hospital or home health setting.
• Current driver’s license and access to a dependable vehicle.

Responsibilities

• Makes the initial nursing evaluation visit.
• Continually evaluates the patients' nursing needs.
• Initiates and coordinates the plan of treatment and necessary revision; instructs and supervises the patient’s family in care and maintenance of a healthy environment.
• Provides services requiring substantial specialized nursing skill.
• Renders nursing care to homebound patients and performs treatments as ordered by the physician.
• Maintains accurate patient history records for supervisor and physician, including drugs and treatments.
• Reports changes in patient condition to the physician and other pertinent personnel.
• Supervises and teaches other personnel providing services to the patient.
• Participates in in-service programs and educational meetings; adheres to agency protocols.
• Participates in staff conferences and the QAPI plan and process.
