Job summary

Well Care Home Health of the Triangle, Inc. is seeking a registered nurse to assess, plan, implement and evaluate patient care for diverse patients in home health settings.

You will manage a caseload, coordinate with a multidisciplinary team, and ensure timely communication according to agency policies. The role requires an active NC or SC nursing license, CPR certification, and 1 year of RN experience with at least 2 years of clinical practice overall; home health experience is preferred.

Qualifications

• Graduate of an accredited school of nursing (AD, Diploma or BSN).
• Current RN license in NC/SC; CPR certification required.
• One year RN experience; total 2+ years clinical experience.
• Home health experience preferred; validates with agency policy.
• Must have valid NC driver's license and a reliable vehicle.
