Job summary

Well Care Home Health of the Triad, Inc. seeks a Registered Nurse to provide home health services across a varied patient population. The role requires planning, coordinating, and evaluating patient care in partnership with a multidisciplinary team.

Applicants must have an active NC/SC license, CPR certification, and at least 1 year of RN experience (2+ years total clinical experience). A valid NC driver’s license and dependable vehicle are essential.

Qualifications

• Graduate of an accredited school of nursing (AD, Diploma, or BSN).
• Current license to practice in NC/SC; CPR certification required.
• Minimum 1 year RN experience; 2+ years clinical experience overall; home health preferred.
• Ability to transfer and guard patients; literate in English; good communication with team.
• Must be able to lift/push/pull objects up to 50 pounds; valid NC driver's license and vehicle.
• Flexible schedule; variable hours including weekends/holidays as needed.

Responsibilities

• Assesses, interprets, plans, implements and evaluates patients per age/diagnosis.
• Manages caseload and coordinates care with multidisciplinary team.
• Communicates according to agency policies and guidelines.
• Contributes to program effectiveness and promotes customer satisfaction.
