Job summary

Well Care Home Health of the Lowcountry, Inc. in Conway, SC is seeking a skilled registered nurse to provide comprehensive home health services to a diverse patient population.

You will assess, plan, implement and evaluate care, coordinating with a multidisciplinary team across ages and diagnoses. The ideal candidate holds an accredited nursing degree (AD, Diploma, or BSN), is licensed in SC/NC with CPR, and has at least 1 year of RN experience (2+ years total clinical).

Qualifications

• Education: Graduate of an accredited school of nursing (AD, Diploma, or BSN).
• Licensure: Current license to practice in SC/NC; CPR certification required.
• Experience: Minimum 1 year RN; total 2+ years clinical experience; home health preferred.
• Technical: Hand/eye coordination; able to use computer; literate in English.
• Interpersonal: Ability to interact positively with patients, families, physicians and staff.
• Physical: Lift/transfer patients; may require frequent pushing, bending and lifting up to 50 pounds.
• Mental: Assess, plan, evaluate care; teach patients about condition/recovery.
• Sensory: Visual and hearing acuity to monitor patient condition.
• Hazards: Exposure to bloodborne pathogens; safe home visits.
• Hours: Variable schedule; weekends/holidays as needed.
• Population Served: Adolescents, adults, geriatrics, pediatrics.
• Driver: Valid NC/SC driver’s license and vehicle required.

Responsibilities

• Assesses, interprets, plans, implements and evaluates patients according to the patient’s age and diagnosis.
• Effectively and efficiently manages the care of a caseload of patients and coordinates care with a multidisciplinary team.
