Qualifications

• Graduate of an accredited school of nursing (AD, Diploma, or BSN).
• Current license to practice professional nursing in SC/NC; CPR certification.
• Minimum 1 year RN experience and total 2+ years in clinical settings; home health preferred.
• Must be able to communicate and be literate in English; ability to transfer and guard patients.
• Valid North Carolina driver’s license and a vehicle.

Responsibilities

• Assess, interpret, plan, implement and evaluate patients according to age/diagnosis.
• Manage a caseload and coordinate care with a multidisciplinary team.
• Communicate information per agency policies and guidelines.
• Contribute to program effectiveness.
• Organize and perform work efficiently.
• Maintain and adjust schedule to enhance agency performance.
