Job summary

Anova Health Care System is seeking a compassionate registered nurse to deliver high-quality home-based care in the Bridgeville, PA area. You will perform admissions, care management, and follow-up visits using the nursing process, build and update the plan of care with the interdisciplinary team, and provide hands-on nursing support at patients'' homes.

Qualifications

• Active RN license in the State of Pennsylvania.
• CPR certification.
• At least 1 year of experience as a registered nurse.
• Strong organizational and communication skills.
• Reliable transportation, a valid driver''s license, and insurance.
• Availability for a minimal on-call rotation, including weekends, evenings, and holidays.
• Prior home health experience preferred.

Responsibilities

• Provide admission, care management, and follow-up visits using the nursing process: assess, diagnose, plan, implement, evaluate.
• Utilize a brand-new AI system to keep documentation efficient and patient care central.
• Build the plan of care for new patients and update it with the interdisciplinary team.
• Deliver hands-on nursing care including assessment, medication management, wound care, IV therapy, and patient education.
• Coordinate care with physicians, PAs, nurse practitioners, home health aides, therapists, and other clinicians.
• Travel to patients'' homes or facilities.
• Keep each patient''s physician updated on care needs and progress.
• Supervise LPNs, home health aides, and personal aides as needed.
• Teach patients and families home care procedures and safety strategies.
• Maintain full compliance with HIPAA and state/federal regulations.
• Document clinical notes clearly and concisely.
