Job summary

UnitedHealth Group in Danville, PA is seeking a Registered Nurse to deliver and coordinate nursing care in patients' homes, collaborating with physicians and the care team to maintain continuity of care.

You will supervise home health aides, conduct assessments, and ensure care plans meet State guidelines. The role offers flexible schedules and mileage reimbursement as part of a comprehensive benefits package.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Preferred: CPR Certification.
• Preferred: 1+ years of Home Health experience; ability to work independently; strong communication and organizational skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with physicians and other health professionals.
• Supervises home health aides and licensed staff; provides instruction and assigns tasks per regulations.
• Performs supervisory visits and ensures adherence to care plans.
• Documents visits per policy; completes assessments and medication reconciliation; ensures timely documentation.
• Communicates with physicians, patients, families, and staff to coordinate care and improve outcomes.
