Job summary

Vivie is seeking a dedicated Home Health RN to provide direct client care in patients' homes, educating families, and ensuring safe, compliant nursing across the community.

You will document services in Vivie's EMR, coordinate with LPNs and HHAs, travel to client homes, and participate in team meetings. Minnesota RN license, CPR within 6 months, valid driver's license, English proficiency, and ability to pass a background check are required.

Qualifications

• Active Minnesota RN license required.
• CPR certification within 6 months of hire preferred.
• Valid driver’s license and access to insured vehicle.
• Ability to pass state-mandated background check.
• Ability to read, write, and speak English proficiently.

Responsibilities

• Provide direct client care and initiate preventive and rehabilitative nursing procedures.
• Maintain accurate client records and billing using Vivie’s EMR system.
• Ensure regulatory compliance and high-quality care per state/federal regulations.
• Communicate effectively with clients, families, physicians, and the care team.
• Participate in team meetings and assist in other areas as needed.
