Job summary

Ascension at Home together with Compassus is seeking a Home Health Registered Nurse (RN) to support patient recovery at home, helping individuals stay independent and enjoy quality time with family.

You will educate patients and families, provide case management and clinical oversight to an interdisciplinary team, and contribute to a caring, inclusive culture that values every teammate.

Qualifications

• RN license in Tennessee.
• Two years of RN experience; home health preferred.
• Current CPR and BCLS certification.
• Valid driver's license.
• Able to work independently in patients' homes.

Responsibilities

• Support patient recovery at home and enable independence.
• Educate patients and families on disease management.
• Case manage and provide clinical oversight to an interdisciplinary team.
• Coordinate care with other providers.

Skills

Two years of RN experience

Home health experience preferred

Current CPR/BCLS certification

Valid driver's license

Education

RN license in Tennessee
