Ascension at Home together with Compassus is seeking a compassionate Home Health Registered Nurse (RN) to support patient recovery at home in Indiana. You will coordinate care with an interdisciplinary team and empower families to reach health goals.

Qualifications

• Active RN license in the state of Indiana.
• Two years of registered nurse experience; home health experience preferred.
• Current CPR and BCLS certification.

Responsibilities

• Support patient recovery and help patients stay at home with quality life.
• Educate patients and families on disease management and self-care.
• Case manage and provide clinical oversight to the interdisciplinary team.
