Classic Home Care, a nurse-owned skilled home health agency serving Southeast Michigan, seeks dedicated Home Health RN to join our Northville-based team.

As a Full-Time RN, you will provide in-home care, start with initial assessments, collaborate with physicians and the interdisciplinary team, educate patients and families, and contribute to continuous service improvement in a supportive, professional environment.

Qualifications

• Active Michigan RN license.
• Reliable transportation.
• Valid driver's license and auto insurance.
• Home health care experience preferred.

Responsibilities

• Provide care to patients in the community to help them regain independence.
• Conduct and complete initial assessment of our patients to generate a personalized plan of care in collaboration with physicians and the interdisciplinary team.
• Observe signs and symptoms and report changes to the physician or health professionals as needed.
• Teach, supervise and counsel the patient and family on home nursing procedures.
• Exhibit critical thinking to improve services and the agency.
