Job summary

Team Select Home Care is hiring private duty nurses to deliver skilled in-home care for medically complex children and adults. You’ll work in patients’ homes with flexible days and nights and a supportive team around you.

Responsibilities include developing individualized care plans, coordinating with physicians and therapists, documenting visits in our EMR, and educating families on home health strategies. Competitive pay and ongoing training are provided.

Qualifications

• Active RN license and CPR certification.
• Clinical experience preferred, but new grads welcome.
• Excellent communication and critical thinking skills.
• Love for patient-centered home-based care.
• Confident, compassionate, and calm in routine and critical moments.

Responsibilities

• Care for medically complex patients at home.
• Coordinate care plans with physicians, therapists, and team.
• Monitor patient progress and update care plans.
• Educate and support families with in-home care techniques.
• Document visits and care notes in EMR.
• Collaborate with clinical supervisor and participate in case conferences.
• Be part of a long-term care team focused on safety and continuity.
