Job summary

Team Select Home Care is seeking a Registered Nurse (RN) to deliver one‑on‑one in‑home care to medically complex children and adults. You’ll collaborate with physicians and families to ensure safety, comfort, and continuity of care in the home setting.

Responsibilities include developing and updating care plans, documenting visits, and participating in case conferences with the clinical team. Flexible scheduling and ongoing education opportunities are offered.

Qualifications

• Active RN license and CPR certification are required.
• Experience in home health or pediatric care is a plus.
• Strong communication and critical thinking skills are essential.

Responsibilities

• Provide private duty nursing care in patients' homes.
• Coordinate care plans with physicians, therapists and family.
• Document visits and care notes in EMR.
