Job summary

Team Select Home Care is seeking a Registered Nurse (RN) to provide skilled in-home care for medically complex children and adults. You will deliver one-on-one nursing care in patients' homes, coordinating with a clinical team and documenting in the EMR.

The role requires an active RN license, CPR, and strong communication skills. Home health experience is preferred; new grads are welcome. Flexible scheduling and weekly pay are offered to support work-life balance.

Qualifications

• Active RN license and CPR certification.
• Experience in home health care preferred; willing to train passionate new grads.
• Excellent communication and critical thinking.

Responsibilities

• Develop, implement, and evaluate individualized patient-care plans under physician guidance.
• Coordinate care with physicians, therapists, and other team members.
• Document visits and care notes in the EMR.
• Educate families on home health care techniques and safety.
• Attend case conferences and participate in on-call coverage as needed.
