Job summary

Aspire Home Healthcare is seeking Registered Nurses (RNs) to provide direct patient care and case management in home health settings in Ohio. Opportunities exist for flexible scheduling and part-time roles, with per-visit pay starting at $55 and mileage reimbursement.

Join a compassionate team dedicated to quality, one-on-one care in patients' homes, with orientation at the Dublin office and ongoing performance reviews.

Qualifications

• Active RN license in OH.
• Experience with OASIS and WellSky (Kinnser) strongly preferred.
• Med-surg and wound care experience a plus.
• Current TB test (within last 12 months) and CPR certification.

Responsibilities

• Obtain medical history and assess patient condition and needs.
• Conduct physical assessments, including vitals and mobility evaluations.
• Educate patients and families on care plans and home health services.
• Deliver skilled nursing care while maintaining HIPAA compliance.
• Document and submit accurate visit records promptly.
• Develop and update individualized care plans as needed.
• Coordinate with LPNs, Home Health Aides, and Personal Care Attendants.
• Facilitate referrals for additional services such as therapy or medical social work.
• Evaluate the effectiveness of nursing services and adjust as appropriate.
