Qualifications

• Active CA RN license required.
• Experience with home health and case management preferred.
• CPR certification and current TB test required.

Responsibilities

• Provide direct patient care and case management per physician orders.
• Develop and update care plans with interdisciplinary team.
• Obtain medical history and assess patient condition.
• Perform vitals, wound care, and meds administration as needed.
• Document visits promptly and maintain HIPAA compliance.
• Coordinate referrals to therapy and social work.
