Job summary

The Lakes Home Care is seeking a dedicated Registered Nurse to join our Home Health team in Florida. This role focuses on applying the nursing process to deliver patient care, with clinical judgment under the direction of the Director of Clinical Services.

Responsibilities include comprehensive assessment, planning, implementation and evaluation of care, documenting care plans, and coordinating with health care team to meet patient needs. Flexible hours and rewarding home-based setting offered.

Qualifications

• Current RN license in the state of practice.
• BLS certification or equivalent.
• One year of RN experience preferred.
• Strong assessment and documentation skills.

Responsibilities

• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
• Collects, analyzes, and interprets data from health care members and documents nursing diagnoses.
• Documents the patient's plan of care with diagnoses, outcomes and interventions.
• Revises the plan of care based on evaluation and medical changes.
• Apply clinical judgment in evaluation activities and establish priorities.
• Perform other RN duties as assigned.
