Job summary

The Lakes Home Care in Florida is seeking a dedicated Registered Nurse to join our Home Health team, responsible for applying the nursing process and delivering high-quality patient care under the Director of Clinical Services. The RN will demonstrate clinical judgment and autonomous decision-making in a home-care setting.

Responsibilities include assessing, planning, implementing, and evaluating patient care, documenting plans of care, and prioritizing interventions.

Qualifications

• Current RN license required for Florida.
• Experience in home health or similar setting preferred.
• BLS certification required.

Responsibilities

• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collects, analyzes, and interprets data from health care team and documents nursing diagnoses.
• Document the patient’s plan of care with expected outcomes and interventions.
• Performs interventions according to priorities, plan of care, and policies.
• Revises the plan of care based on evaluation and changes in medical plan.
• Uses clinical judgment to meet patient care needs on an assigned unit/floor.
• Other RN duties as assigned.
