The Lakes Home Care in Florida is seeking a dedicated Registered Nurse to join our Home Health team. The RN will apply the nursing process and deliver patient care under the direction of the Director of Clinical Services.

Responsibilities include assessing, planning, implementing, and evaluating care, documenting plans of care, and performing interventions while maintaining priority-patient safety. Flexible scheduling and a rewarding home-care environment are emphasized.

Qualifications

• Current RN license for the state of practice.
• Current Health Certificate (per facility contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
• One year prior RN experience preferred.

Responsabilités

• Utilizes the nursing process to assess, plan, implement and evaluate patient care.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient's condition.
• Collects, analyzes, and interprets data from health care members and documents nursing diagnoses.
• Document the patient's plan of care with nursing diagnoses, expected outcomes, and interventions.
• Perform interventions according to priorities, plan of care, and hospital policies.
