Job summary

Interim HealthCare, the nation’s first home care company, is seeking a Home Health Registered Nurse in Jefferson, WI. You will provide home-based care to patients with illnesses and chronic conditions, coordinate with physicians and supervise LPNs, CNAs and aides to deliver personalized care.

Ideal candidates have an active WI RN license, at least 1 year of nursing experience (preferably in home health), CPR certification, and a commitment to compassionate, high-quality care.

Qualifications

• Graduate of an accredited nursing program with active WI RN license.
• Minimum of 1 year nursing experience, ideally in home healthcare.
• CPR certification is required.

Responsibilities

• Provide home-based care to patients with illnesses, injuries, and chronic diseases.
• Oversee LPNs, CNAs and aides providing patient care.
• Educate patients on plan of care, medications, therapy, diet and exercise.
• Coordinate discharge from services.
