Job summary
Interim HealthCare of Green Bay is hiring Home Health Registered Nurses to provide one-on-one, home-based care in Shawano, WI and surrounding areas. You’ll work with a physicians team, oversee LPNs, CNAs, and aides, and help patients recover and stay independent at home.

Qualifications
- Graduate of an accredited nursing program and WI RN license.
- CPR certification.
- Knowledge of state and federal home health regulations.
- Excellent communication and compassionate care skills.
Responsibilities
- Provide home-based care to patients in Shawano, WI and surrounding areas after hospital, rehab, or nursing facility stays.
- Oversee LPNs, CNAs and aides delivering patient care as part of a care team.
- Assess patients, document progress, and report changes to the physician.
- Educate patients and families on plan of care, medications, therapy, diet and exercise.
- Coordinate discharge planning and ensure goals are met.
