Job summary

Interim HealthCare is seeking a Licensed Practical Nurse (LPN) to join our Janesville home health team. The role covers the Janesville territory with day shifts, focusing on high-quality, patient-centered care in home settings.

You will assess patients, administer medications, coordinate with an interdisciplinary team, educate families, and ensure equipment and supplies are available. Wisconsin LPN license and CPR are required, with at least 1 year of nursing experience in hospice/home health

Qualifications

• Graduate of an accredited nursing program with active LPN license in Wisconsin.
• Minimum of 1 year nursing experience, ideally in hospice or home healthcare.
• CPR certification and knowledge of home health regulations.

Responsibilities

• Provide home-based care to patients with focus on quality of life.
• Work with an interdisciplinary team to carry out the patient’s plan of care.
• Perform patient assessments, check vitals, manage medications and treatments.
• Ensure accessibility of medical equipment and supplies; educate family on prognosis and care plan.
