Job summary

Elite Home Health, part of LHC Group and aligned with UnitedHealth Group, seeks a Registered Nurse to provide and direct nursing care in patients' homes. The role involves coordinating total care plans with physicians and the health team to ensure continuity of patient care and adherence to policies.

The RN will supervise aides, complete OASIS assessments, document visits, and communicate effectively with physicians and families to deliver high-quality care.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current driver's license and access to a dependable vehicle or public transportation.

Responsibilities

• Initiates, develops, and revises the plan of care with the physician and health care team.
• Supervises home health aides and LPNs; provides instruction and assigns tasks according to state and federal regulations.
• Provides required supervisory visits.
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Documents patient visits per policy and payer requirements; syncs timely per agency policy.
• Communicates with physicians, patients, and family to ensure quality care and service excellence.
• Participates in on-call and weekend rotation as needed to meet patient needs.
• Adheres to and participates in the agency's utilization management model.
