Job summary

UnitedHealth Group is seeking a Registered Nurse to provide and direct nursing care for patients in home health settings. You will coordinate total plans of care, collaborate with physicians, and support continuity of patient care while ensuring compliance with laws and agency policies.

The role requires valid RN licensure, a driver’s license, and ability to travel to patients’ homes. Preferred candidates have 1+ year of home health experience and strong communication skills.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification or ability to obtain within 90 days of hire.
• 1+ years of Home Health experience (preferred).
• Ability to work independently with strong communication and organizational skills.

Responsibilities

• Initiates and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care by home health aides and licensed nurses; provides instruction and assigns tasks per regulations.
• Provides high-quality clinical nursing services and completes assessments (OASIS) and medication reconciliation.
• Documents visits and coordinates with the care team to ensure continuity of care.
• Participates in on-call and weekend rotation as needed and supports performance improvement efforts.
