Job summary

Louisiana HomeCare of New Orleans, a part of LHC Group, within the Optum family, seeks an RN in Home Health to provide and direct nursing care in patients' homes and coordinate the overall plan of care with the physician and health care team.

The role includes supervising home health aides, ensuring high-quality documentation, and conducting assessments (OASIS). Must have RN licensure, a valid driver's license, and meet state requirements.

Qualifications

• Current and unrestricted RN licensure in state of practice
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Willingness to comply with applicable laws, regulations, and agency policies

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals
• Supervises care provided by home health aides and licensed nurses, provides instruction, and assigns tasks according to regulations
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely
• Documents patient visits per policy and payer requirements, including coordination with the care team
• Participates in on-call and weekend rotation to meet patient needs
