Job summary

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification preferred or within 90 days of hire.

Responsibilities

• Develop, implement, and revise the plan of care in collaboration with physicians and health care professionals.
• Supervise home health aides and licensed nurses; provide instruction and assign tasks.
• Perform required supervisory visits and ensure compliance with state and federal regulations.
• Complete comprehensive assessments (OASIS) and medication reconciliation accurately.
• Document patient visits per policy and payer requirements; communicate with physicians and care team.
• Coordinate communication with patients, families, and community resources to ensure quality care.
