Job summary

Liberty Home Care in North Carolina is seeking an experienced Home Care Registered Nurse (RN) to join our patient-focused team. You will participate in care planning, deliver nursing services per the care plan, and collaborate with interdisciplinary colleagues to promote recovery at home.

Requirements include RN licensure in NC, graduation from an approved nursing program, at least one year of clinical experience, CPR certification, strong communication, and computer skills.

Qualifications

• Successful completion of nursing education from an approved school of nursing.
• Current RN licensure in North Carolina.
• At least one year of clinical nursing experience.
• CPR certified.
• Excellent communication skills.
• Ability to use computer applications.

Responsibilities

• Actively participates in the interdisciplinary care planning process.
• Provides professional nursing services per the plan of care established by the Patient Care Coordinator.
• Promotes favorable outcomes through collaborative practice patterns and the appropriate and cost-effective use of resources.
• Documents accurately, thoroughly, and concisely to demonstrate evidence of delivered home health services, to facilitate coordination and reimbursement, and to ensure compliance with licensure and accreditation requirements.
• Demonstrates positive and effective interpersonal relationships.
• Demonstrates personal accountability for accomplishing work assignments and for professional growth.
