Job summary

Liberty Home Care in North Carolina seeks an experienced Registered Nurse to provide in-home nursing services under the plan of care coordinated by Patient Care staff. You will participate in interdisciplinary care planning and ensure high-quality, cost-effective home health solutions for patients in their homes.

The ideal candidate holds an active NC RN license with at least one year of clinical experience; CPR certification is required, and home health experience is preferred but not mandatory.

Qualifications

• Successful completion of nursing education from an approved school of nursing.
• Current RN license in NC with at least 1 year of clinical nursing experience (acute or long-term).
• Home health experience preferred but not required.
• CPR certification.
• Knowledge of medical record keeping and adult learning principles.
• Ability to communicate effectively and use computer applications.
• High-level problem-solving, critical thinking and reasoning for patient care planning.

Responsibilities

• Actively participates in the interdisciplinary care planning process.
• Provides professional nursing services in accordance with the plan of care established by the Patient Care Coordinator.
• Promotes favorable outcomes through collaborative practice patterns and the appropriate and cost-effective use of resources.
• Documents accurately and concisely to demonstrate delivered home health services and support quality improvement.
• Demonstrates positive and effective interpersonal relationships.
• Demonstrates personal accountability for work assignments and professional growth.
