Job summary

Gentiva is seeking a compassionate Registered Nurse to provide home-based clinical care to patients and families. You will perform assessments, develop and update care plans, administer medications, and educate patients and caregivers on disease management at home.

You will coordinate care with an interdisciplinary team to ensure dignity, comfort, and quality outcomes while working in a mission-driven, patient-centered environment that values empathy and teamwork.

Qualifications

• Active Tennessee RN license.
• Minimum 1 year of home health or patient-facing care.
• Strong communication and care planning skills.

Responsibilities

• Perform comprehensive home assessments and develop individualized care plans.
• Administer medications and treatments per physician orders.
• Educate patients and caregivers on disease management and safety in the home.
