Job summary

Mercy is seeking a registered nurse to provide home health care on a per diem basis in Yukon, OK. Responsibilities include patient assessment, care coordination, IV therapy, and clinical documentation using the Epic system.

The role supports the home health program and requires flexibility for travel within the assigned area. The ideal candidate will have an OK RN license, CPR certification, and at least three years of nursing experience, with hospital or home health exposure preferred.

Qualifications

• RN licensed in OK state.
• Graduation from an accredited nursing program.
• CPR certification required; recert biannually.

Responsibilities

• Provide day-to-day nursing care in patients' homes.
• Perform case management and total care for multiple patients with varying acuity.
• Administer IV therapy and assist with care tasks.
• Document using Epic in a timely, accurate manner.
• Per diem visits as needed per department policies.
