Job summary

Mercy is seeking a Home Health RN for per diem visits across the Mustang, OK area. The role focuses on direct patient care, case management, and coordination with multidisciplinary teams.

You will document care in Epic, perform assessments, and support safe transitions from hospital to home. The position requires RN licensure in the state, CPR certification, and the ability to lift 50 pounds and transport patients as needed.

Qualifications

• Graduation from an accredited school of nursing.
• RN license in the applicable state with a valid driver’s license and automobile liability insurance.
• Three years nursing experience with at least one year in a hospital setting; home health experience preferred.
• CPR certification (biannually).

Responsibilities

• Provides day-to-day patient care in the home setting.
• Per Diem visits as needed following department policies.
• Documentation in the Epic system.
• Assist with care coordination, education, and safety measures for patients and families.
