Qualifications

• RN license in applicable state with valid driver's license.
• Three years nursing experience; at least one year in hospital; home health preferred.
• CPR certification biannually.
• Ability to lift 50 pounds and transfer patients weighing 100 pounds.

Responsibilities

• Provide day-to-day home nursing for multiple patients with varying acuity.
• Perform assessments, care planning, and Epic-based documentation.
• Administer IV therapy, wound care, and patient teaching.
• Coordinate with the care team and assist with community referrals as needed.
