Job summary

Mercy seeks a dedicated Home Health RN for per diem visits in Guthrie, OK. You will deliver comprehensive patient care in the home, manage admissions and assessments, and document thoroughly in Epic.

Qualifications

• Graduation from accredited school of nursing.
• RN license in applicable state of practice.
• Three years nursing experience with minimum one year in a hospital setting, then preferably in home care or the equivalent.
• CPR biannually.

Responsibilities

• Provide day-to-day patient care in the home setting per policy and procedure.
• Perform admissions, assessments, recertifications, and discharges as required.
• Document using the Epic system in a timely and accurate manner.
• Assist with other needs to ensure safety and quality care in the home, including lifeline, meals on wheels, and public program instructions.
