Job summary

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises home health aides and licensed nurses, provides instruction, and assigns tasks per regulations.
• Provides supervisory visits and ensures compliance with care plans and policy.
• Completes comprehensive assessments and medication reconciliation accurately and timely.
• Documents visits per policy and payer requirements, and coordinates with care team.
