Job summary

UnitedHealth Group is seeking a Nurse Case Manager to guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning, and managing ongoing outpatient care under supervision of an RN or MD.

You will educate, coach, and support decision-making, collaborate with a Care Advocate and Social Worker, supervise LPNs and HHAs, and participate in clinical development and continuing education programs.

Qualifications

• Active RN license in the state(s) of assignment.
• At least 1 year of clinical RN experience.
• Current CPR certification.
• Valid driver''s license and reliable transportation.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other disciplines as indicated by the patient''s identified needs or documents rationale for not doing so.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform on-call responsibilities and on-call services to patients/families as assigned.
• Participate in clinical development and continuing education programs.
• Other duties as assigned.
