Advanced Home Health and Hospice is seeking a dedicated Home Health RN Case Manager to provide high-quality care to patients in their homes across Lehi to Provo.

The role involves coordinating care plans, educating families, and collaborating with healthcare teams to optimize outcomes. Strong assessment skills, excellent communication, and proficiency with EHRs are essential for success in this full-time weekday position.

Qualificações

• Current RN license in state of practice.
• BSN preferred.
• 1+ years in home health or case management.
• Strong clinical assessment and critical thinking.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time.
• Proficient in EHR and healthcare tech.
• Compassionate, patient-centered care.

Responsabilidades

• Conduct comprehensive patient health assessments in home setting.
• Develop, implement, and evaluate individualized care plans with patients and families.
• Monitor progress and adjust plans as needed.
• Provide direct nursing care including meds, wound care, and health education.
• Coordinate with interdisciplinary teams for continuity of care.
• Educate patients/families on disease management and adherence.
• Maintain accurate documentation of care and progress.
• Ensure compliance with healthcare regulations and standards.
