Safe Life Home Health Care, LLC is seeking a skilled Home Health RN Case Manager with a minimum of 1 year home health experience and proficiency in WellSky Kinnser EMR to join our 4-star team. You will prioritize patient health and help prevent hospitalization while delivering exceptional care in patients' homes.
Safe Life utilizes Andy AI to assist with OASIS completion and submission within 24 hours, supporting clinicians in timely care delivery.
Competenze
- Valid Illinois RN license.
- CPR Certification.
- Minimum 1 year home health experience.
- Proficiency in WellSky Kinnser EMR.
- Valid driver's license and auto insurance.
- Strong clinical assessment and communication skills.

- Provide strong clinical skills, excellent communication abilities, and a passion for patient-centered care.
- Conduct comprehensive assessments of patients' health status and needs in their home environment.
- Develop and implement individualized care plans based on patient assessments and medical history.
