Safe Life Home Health Care, LLC is seeking a Home Health Registered Nurse Case Manager with a minimum of 1 year home health experience and proficiency in Well Sky Kinnser EMR to join our 4-star team. Prioritize patient health and help prevent hospitalization while providing exceptional care in the comfort of patients' homes.

Safe Life utilizes Andy AI, which assists our clinicians with OASIS completion and submission within 24 hours. Expected Hours 20 – 40 per week; Pay $45.00 - $50.00 per hour.

Qualifications

Illinois RN license required.
CPR certification required.
Valid driver’s license and auto insurance.
Minimum 1 year home health experience.
Proficiency in Well Sky Kinnser EMR.
Strong clinical assessment and communication skills.
Compassionate and patient-centered approach.

Responsibilities

Provide in-home nursing care and case management.
Coordinate care to prevent hospitalization and ensure quality outcomes.
Document care using EMR and coordinate with care teams.
