Job summary

This Per Diem position offers visits on an as-needed basis, aligned with department policies and Mercy Service Standards.

Qualifications

• Graduation from accredited nursing school.
• Current state licensure.
• CPR certification.
• One year experience in a related field preferred.

Responsibilities

• Provide day-to-day nursing care in patient homes.
• Perform case management and admissions to home health services.
• Conduct post-hospitalization assessments, recertifications and discharges.
• Document care using the Epic system in a timely and accurate manner.
