Job summary

Freeman Home Care is seeking a full-time Home Health Registered Nurse to deliver direct patient care in patients’ homes throughout designated regions. You will conduct ongoing evaluations, review charts, identify variations in care, and collaborate with families, physicians, and providers to ensure high-quality, compliant care.

The role requires an active MO/KS or compact-state RN license, BLS, clean driving record, and successful background checks.

Qualifications

• Active RN license in MO/KS or compact state
• BLS for Healthcare Providers within 30 days of hire
• Clean driving record and valid driver's license
• Criminal background checks and safety registry requirements

Responsibilities

• Ongoing objective evaluation of patient care through chart reviews and data to identify variations in care.
• Coordinate with patients, families, physicians and regulatory bodies to ensure compliant care.
• Maintain effective communication with interdepartmental staff and external agencies including Medicare/Medicaid.
