Job summary

Freeman Health System is seeking an RN Case Manager for Home Health in Kansas. This full-time role focuses on continuous evaluation of patient care, chart reviews, and coordinating with medical staff, families, and outside agencies to ensure regulatory compliance.

The position requires an RN license in MO/KS or a compact state, BLS, a valid driving record, and successful KDADS and Family Care Safety Registry checks. Join Freeman’s Home Care team delivering quality in-home care.

Qualifications

• AHA BLS for Healthcare Providers within 30 days.
• Auto liability insurance.
• Clean driving record.
• Current driver's license.
• Current Missouri and Kansas Registered Nurse license or current Registered Nurse license from a compact state. If a compact is held, it must be in the nurse's state of residence.
• Must be able to successfully pass KDADS criminal record check upon hire.
• Must register with and successfully pass the Family Care Safety Registry upon hire.

Responsibilities

• Provides an ongoing objective evaluation of the important aspects of patients care through regular review of patient charts, and identification of variations in care to provide data for use in the correction of potential problems and to meet the requirements of all regulatory agencies.
• Relates effectively with interdepartmental and intradepartmental personnel, medical staff, patients, patients families, and outside agencies, including Medicare, Medicaid, and other regulatory and third party payers.
