Job summary

Community Hospice and Palliative Care is seeking an RN Case Manager to plan and manage nursing care for an assigned caseload in home or long-term care settings, focusing on a holistic, patient-centered approach.

In collaboration with the interdisciplinary team, you will educate patients and families, coordinate care across sites, maintain accurate documentation, and participate in quality improvement initiatives. Flexibility in work location across care settings is required.

Qualifications

• Minimum of one year of RN experience.
• Preferred: one year in hospital medical-surgical or oncology.
• Alternative: five years of LPN experience with hospital MS/oncology in past five years.

Responsibilities

• Assess patient and family goals and needs.
• Collaborate with interdisciplinary team to develop care plans.
• Participate in team meetings and ethical discussions.
• Educate patients and caregivers about disease progression, medications, nutrition, and hospice philosophy.
• Coordinate timely and effective responses to changing or urgent patient needs.
• Maintain accurate, timely, and compliant documentation.
• Support and educate team members, patients, families, and the broader community.
• Participate in internal quality improvement initiatives.
• Facilitate interdisciplinary group (IDG) meetings in the absence of the manager.
• Perform other duties as assigned.
