Amedisys is seeking a dedicated Registered Nurse Case Manager to join our patient-centered home health team in Pensacola, FL. You will perform patient assessments, collaborate with the care team to develop and implement plans of care, and help patients heal in the comfort of their homes.

Responsibilities include supervising LPNs and HHAs, timely documentation, and ongoing clinical education for staff. We offer competitive pay and comprehensive benefits to support work-life balance.

Formación

• 1+ year of clinical experience as a Registered Nurse (RN).
• Current RN license, specific to the state(s) assigned.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• RN intern program if <6 months RN experience.

Responsabilidades

• Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
• Makes referrals to other disciplines as indicated by patient needs or documents rationale.
• Promotes patient health and independence through teaching and rehabilitative measures.
• Supervises LPNs and HHAs.
• Completes documentation timely and accurately.
• Communicates patient progress to the clinical manager and care team.
• Plans and provides staff education.
• Performs on‑call responsibilities and services as assigned.
• Participates in clinical development and continuing education.
